In the opening quarter of 2026, Tealwood Enterprise, Inc. owned Woodlyn Heights Healthcare Center, as indicated by data from the Centers for Medicare and Medicaid Services (CMS). The center, based in Dakota County, has operated under Tealwood Enterprise, Inc. since May 31, 2013.

The facility operates as a for-profit nursing home.

For this period, Woodlyn Heights Healthcare Center earned an overall CMS rating of two stars, lower than Minnesota’s state average nursing home rating of 3.2. CMS evaluates such ratings based on health inspection results, staffing resources, and the overall care delivered to residents.

The data also shows the center cared for an average of 64.1 residents per day in the first quarter of 2026, out of 79 available beds. Currently, the nursing home is not affiliated with a hospital.

During that same timeframe, the center incurred no fines or penalties, according to CMS records.

The U.S. Census Bureau reports the population of older adults in the United States continues to expand, even as younger age groups experience a decline. Between 2020 and 2024, Americans age 65 and older rose by 13%, while those under 18 decreased by 1.7%. Over the last century, the older adult demographic increased sharply, from 4.9 million in 1920 to over 61 million in 2024.

A study from 2021 estimated nearly half of the older adult group—about 28 million—will need long-term services and support, with more than a third potentially requiring nursing home care in the future.

“Living a long life is something that many of us want and could get,” said Jesse Slome, executive director of the American Association for Long-Term Care Insurance. “But when we live a long life, the chances of us needing long-term care increase exponentially. But when you need that type of care, there are limited options.”
